This notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Our Commitment to Your Privacy

Community Care - Behavioral Health is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with this Notice of Privacy Practices. We are committed to protecting your health information and will use it only as permitted by the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law.

How We May Use and Disclose Your Health Information

We may use and disclose your PHI for the following purposes:

  • Treatment: To provide, coordinate, or manage your healthcare and related services.
  • Payment: To obtain payment for services we provide to you, including billing insurance or other payers.
  • Healthcare Operations: For activities such as quality improvement, staff training, and business planning.
  • As Required by Law: When required by federal, state, or local law.
  • Public Health and Safety: For public health activities, reporting abuse or neglect, or to prevent a serious threat to health or safety.

Uses and Disclosures Requiring Your Authorization

For most uses and disclosures of your PHI beyond treatment, payment, and healthcare operations, we will obtain your written authorization. You may revoke an authorization in writing at any time.

Your Rights Regarding Your Health Information

You have the following rights:

  • Right to Access: You may request to inspect or obtain a copy of your health records.
  • Right to Amend: You may request that we amend your health information if you believe it is incorrect or incomplete.
  • Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.
  • Right to Request Restrictions: You may request restrictions on how we use or disclose your PHI for treatment, payment, or healthcare operations.
  • Right to Request Confidential Communications: You may request that we communicate with you in a certain way or at a certain location.

Special Protections for Substance Use Records

Federal law (42 CFR Part 2) provides additional protections for substance use disorder treatment records. We will not disclose such records without your written consent except as permitted by law.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Changes to This Notice

We reserve the right to change this Notice. We will post the revised Notice at our facilities and on our website. A copy is available upon request.
